components:<\/li><li><b>Component 1:<\/b> Diabetes Educator Nursing Officers (DENO)<\/li><li><b>Component 2:<\/b> Primary\u2013Tertiary Healthcare Partnership Model<\/li><li><b>Component 3:<\/b> Diabetes Prevention through Health Promotion<\/li><\/ul>\t\t\t\t\t\t\t\t<\/div>\n\t\t\t\t<div class=\"elementor-element elementor-element-9c2e982 elementor-widget elementor-widget-text-editor\" data-id=\"9c2e982\" data-element_type=\"widget\" data-e-type=\"widget\" data-widget_type=\"text-editor.default\">\n\t\t\t\t\t\t\t\t\t<ul><li>The project focused on capacity building of medical and allied health professionals, with emphasis on patient follow-up and self-care practices.<\/li><li>Strengthened long-term diabetes care within Primary Health Care (PHC) settings.<\/li><li>Established a community-based health promotion model to support primary prevention of diabetes.<\/li><li>Encouraged participation from both formal sectors (health, education, etc.) and informal sectors (community groups) in suburban areas.<\/li><li>Each component had clearly defined activities, implemented simultaneously with planned timelines and budgets.<\/li><li>The project was led by three coordinators, contributing their expertise on a voluntary basis.<\/li><li>Teams included representatives from administrative, academic, and healthcare sectors.<\/li><li>Activities were carried out with active involvement of Diabetes Prevention Task Force (DPTF) members.<\/li><li>Key stakeholders such as the Sri Lanka Medical Association (SLMA), Ministry of Health, WHO, and Health Promotion Foundation acted
